Biography:
Amy Kulak, MD is a Board Certified Oculofacial Plastic Surgeon and ophthalmologist, specializing in cosmetic and reconstructive eyelid surgery, tearing disorders, and ophthalmology. She is a fellow of the American Academy of Cosmetic Surgeons, and member of American Academy of Ophthalmology. Her highly specialized training in both oculofacial plastics and ophthalmology allows her to understand the importance of the functional aspect of the eyelid and upper face while performing cosmetic procedures.

She earned her undergraduate degree from Brown University and then pursued clinical research interests at Weil Cornell Medical Center in New York City. Dr Kulak received her medical degree from the University of Miami-Miller School of Medicine followed by residency in ophthalmology at SUNY Downstate Medical Center. She further specialized her training and completed a fellowship in oculofacial plastics training at Beaumont Medical Center, The Kresge Eye Institute, and at the Consultants in Ophthalmic and Facial Plastic Surgery in Michigan before settling in Jacksonville, FL.

Dr. Kulak is dedicated to individualizing patient care to help achieve personalized, natural results and utilizes the most current non-invasive technologies and products for facial rejuvenation including Botox®, facial fillers and fractional CO2 laser resurfacing.
